第二回　令和３年度（2021年）学生向け定期健康診断を以下の通り実施します。

■対象学生■
すべての大学院生（正規生：修士課程、博士課程　非正規生：研究生）

※教職員の身分を兼ねる学生は職員健康診断を優先受診してください。検診後、2021年1月から12月までの健診結果（コピー可）を大学院係まで提出してください。 提出期限は令和4年（2022年）1月下旬までです。
※放射線業務従事及び有害物質を取り扱う予定の学生は、「特殊健康診断（第一回：5月10日～5月12日）」に加え、今回の「定期健康診断」は必須です。

■実施日■
2021年（令和3年）4月13日（火）　～　4月28日（水）

 ※必ずマスクの着用をお願い致します。

■予約方法■　
4月10日（土）から予約開始。 　　　　　　
名大ID/PWを使用して、予約システムへログイン。

→　予約完了後、「健診受付票」をダウンロードし、印刷またはスマホの画面を当日受付で提示

■検査内容■  身長・体重・胸部X線・血圧・会話域聴力・健康個人調査　※コロナウィルス感染症防止のため視力検査は実施しません。 　　　　　
　　　　　　  尿検査・・・検査キットは当日保健管理室で配布。各自後日（5/14と5/17のどちらか午前中）保健管理室へ提出。鶴舞キャンパスは、学務課隣の保健管理室分室前で回収。

■会場■　　  東山キャンパス　保健管理室（情報基盤センター隣）　＊予約当日に必ず健診してください。
■持ち物・着用着■    学生証・健康受付票（印刷またはスマホ画面掲示）・両面無地のТシャツ（白以外も可）・マスク着用
■WEBによる検査結果■　2021年6月下旬に保健管理室WEB上で確認できます。　
■健康診断証明書発行■　正規生のみ：2021年6月下旬以降学内自動発行機またはコンビニ発行にて可能。

【外部医療機関で健康診断を受診する学生】  学校保健安全法にもとづき学生定期健康診断の実施が義務付けられております。 学内の定期健康診断を受診せず、外部医療機関で受診した学生は、上記の検査内容の2021年度1月から12月の診断結果（コピー可）をもってこれに代えることができますので、学務課大学院係まで提出してください。この場合は、大学から健康診断書の発行はできません。提出期限は令和4年（2022年）1月下旬までです。

To Target Student

2nd Regular Health Examinations will carry out for current students. Reservation is required.

■Eligible Student■ 
Students :Master Course, Doctoral Course and Reserch Students  
Except to international students who have took the health examination of fall 2020 at University are not necessary to take it at this time. 
*For student planning to be engaged in radiation work and handle harmful substances, this "regular health examinations" is required. Special health examinations will carry out from May 10th to May 12th 2021.　　　　

■Health Examination Schedule■　 April 13th to April 28th 
■How to reserve■　Reservation starts on April 10th
Please use your student ID and PW to log in to the reservation system.

→　After booking,  download "健診受付票(Health reception Slip)" ,  print it or show your screen of your mobile at the reception on the day of reservation. 

■Medical Examination Items■ Physical measurement,  Chest X-ray, blood pressure/pulse, conversational hearing and interview. *Visual acuity test is not performed due to prevent infection. 　　　　　
Urinalysis :  Health Administration Office hand the Urine containers over on examination day. You need to take urine on early morining of May 14th(fri) or May 17th(mon) and submit it on that day when you took the urine at 8:30 - 12:00 to Health Administration Office Branch room next to Student Affairs Office in Tsurumai Campus.  

■Venue for examination■　Health Administration Office at Higashiyama Campus of Nagoya University 
■Belongings■   Student ID card,  健康受付票(Health reception Slip), wear a plain T-shirt (colored T-shirt is possible) 
■Result on the WEB■　You can check it on the end of June 2021
■Certificates■ You can get it from Automatic Machine on each campus or print at convenience store

【For those who have their medical examination at another institution】  Based on the School Health and Safety Law, it is obligatory to carry out regular student health examinations. Therefore, please submit the copy of the result of your health examination to the student affairs office on Tsurumai Campus. It is valid only  when you are taken between January 2021 to December 2021. Once we receive it, we will recognize that you have had the health examination for this year. Nagoya University do not issue a certificate with this result.
