概要
整形外科学は運動器を構成する骨軟骨、筋腱、靱帯、脊髄および末梢神経などの先天性・後天性の疾病や外傷を診療の対象とし、保存治療と外科治療を組み合わせながらその障害の克服を目指す専門領域です。近年超高齢社会に突入した本邦では、健康寿命の延伸に向け運動器障害、いわゆるロコモティブシンドロームの病態解明と治療法の開発が喫緊の課題となっており、その中心的な役割を担う整形外科学分野の重要性が増しています。当研究室は脊椎外科、股関節外科、関節リウマチ外科、小児整形外科、膝肩スポーツ外科、骨軟部腫瘍外科の6つの診療グループから構成されています。各グループは相互に連携しながら日々の診療に携わり、またグループごとに治療手技の創意工夫や開発に努めています。昨今の分子遺伝学や分子生物学、再生医療の長足の進歩によって、今まではアプローチできなかった先天性および変性疾患の病態解明や脊髄損傷、軟骨変性、骨系統疾患などの根本的治療法の実現が夢ではなくなっています。当教室でもこれら難治性運動器疾患の克服に向けたトランスレーショナルリサーチを基礎医学系教室と連携しながら鋭意進めています。
研究プロジェクト
小児整形外科グループ
自家培養骨髄細胞移植と多血小板血漿を併用した骨延長術
骨延長術において、治療期間の短縮と合併症の軽減を目標に幹細胞を用いた細胞治療を独自に開発、臨床経験を重ねてきました。骨延長手術時に骨盤から採取した骨髄液から未分化間葉系細胞を分離、無菌の細胞調整室で3週間培養し骨芽細胞様細胞とします。骨組織を形成する細胞であるこの骨芽細胞様細胞を細胞増殖因子が豊富に含まれた多血小板血漿と一緒に骨延長部に注入します。トロンビン、カルシウムと混和すると多血小板血漿はゲル状に凝固するため、足場となって骨芽細胞様細胞を注入部位に留めます。2011年、この細胞治療は厚生労働省から高度医療に承認されました。この治療によって治療期間は平均2か月短縮し、合併症の発生率は有意に低下しました。

軟骨無形成症の根本的治療開発研究
軟骨無形成症は骨の長軸方向への成長障害により四肢短縮型低身長を呈する疾患です。fibroblast growth factor receptor 3（FGFR3）の恒常的活性化により骨伸長は抑制されることは分かっていますが、根本的な治療法はありません。既存薬のスクリーニングから同定した乗り物酔い止め薬であるmeclozineはFGFR3シグナルを抑制し、疾患モデルマウスにおいて長管骨の成長を促進することを示しました。現在臨床応用に向け、骨伸長促進効果を発揮する有効投与量についてさらなる研究を行っています。

関節リウマチグループ
Tsurumai Biologics Communication Registry: TBCR
関節リウマチにおける生物学的製剤使用症例のデータベース化を目的としてリウマチ診療関連施設と協力し2008年に名古屋大学が中心となって立ち上げたレジストリーである。各々の症例を縦断的に調査している国内有数のレジストリーであり、現在参加施設は18施設、登録症例数は3,000例を超えている。これまでにTBCRを使用した英語論文は20編を超え、臨床試験では得られない、実際の診療に則したデータ、市販後調査では得られない長期にわたるデータを示してきた。

関節リウマチに対するトシリズマブ治療におけるメトトレキサート併用中止後の治療効果持続性に関する研究 (Tocilizumab treatment with Reducing and stopping methotreXate in patients with rheumatoid arthritis in stable low disease activity-state: T-ReX study)
トシリズマブとメトトレキサートの併用療法により治療目標 （寛解または低疾患活動性）を達成した関節リウマチ患者において、メトトレキサートを減量・中止し、トシリズマブ単剤の治療効果がいかに持続するか、治療安全域がいかに広がるかを検討する非盲検非対照多施設共同検証的臨床試験 (UMIN試験ID: UMIN000021247) である。 本研究は研究事務局を名古屋大学医学部附属病院先端医療・臨床研究支援センターに設置し、自施設内の臨床研究支援組織 (ARO: Academic Research Organization) を利用して行う医師主導臨床研究であり、この研究体制は本邦において数少ない試みである。

膝肩スポーツ整形外科グループ
腱障害と血管新生
腱の慢性障害であるTendinopathyでは腱に血管新生を生じることは報告されているが障害との関連は十分に分かっていない。また腱特異的マーカーとしてScleraxis (SCX)やTenomodulin (TNMD)が知られているが、その働きには未だ不明な点が多い。そこで我々は腱障害と血管新生との関係について研究を行っている。前十字靭帯再建術時に採取した腱から得られる培養ヒト腱由来細胞にIL-1βを添加すると、腱特異的マーカーの発現は減少、VEGFなどの血管新生関連遺伝子の発現は上昇し、Scleraxisの発現をknockdownすると、さらに血管新生関連遺伝子の発現は上昇した（膝肩スポーツ整形外科グループ図3；SCXのknockdownにより血管新生関連mRNAの発現が上昇する）。Scleraxisが血管新生に対して抑制的な作用を有すると考えられ、その分子メカニズムの解明を行っている。

Wnt/β-カテニンシグナルによる腱関連遺伝子の発現制御
Wnt/β-カテニンシグナルは組織発生や分化に重要な役割を担っており、損傷腱では活性化することが知られている。我々は腱関連マーカー（Scleraxis、Mohawk、Tenomodulinなど）遺伝子発現におけるWnt/β-カテニンシグナルの影響を調べている。ラット腱由来細胞とヒト腱前駆細胞株において、腱関連マーカーの遺伝子発現はWnt/β-カテニンシグナルの作動薬BIOによって減少し、阻害薬IWRによって増加した。さらにWnt3aによって誘導される遺伝子発現の低下は、シグナル下流の阻害薬IWRによって消失した。Wnt/β-カテニンシグナルによって腱関連マーカー遺伝子の発現は制御されていると考えられ、その分子メカニズムの解明を進めている。

骨軟部腫瘍グループ
デスモイド腫瘍におけるCTNNB1遺伝子解析と治療法の選択
デスモイド型線維腫症の多くは、β-catenin遺伝子（CTNNB1）の変異によりβ-catenin蛋白が核内に蓄積することで発生すると考えられ、変異型によって薬物への反応性や術後再発率は異なることが報告されている。現在デスモイド腫瘍症例には全例CTNNB1遺伝子の変異解析を行っており、今後は変異型・発生部位・年齢別に適切な治療法（保存的内服治療・低用量化学療法・外科的切除）を選択できる治療アルゴリズムの確立を目指している。また次世代シークエンサーによるwhole exome解析や動物モデルの作成など東京大学医科学研究所や愛知県がんセンター研究所との共同研究を行っている。

温熱刺激 (hyperthermia)による骨形成促進
ラット脛骨の骨欠損部にマグタイト（磁鉄鉱）を包埋した脂質 (MCL)を注射し、外部より交流磁場をかけることで生じる温熱刺激 (43-46℃)によって生体内での骨形成が促進されることを世界で初めて発見した。現在臨床応用を目指し、医師主導臨床研究を計画している。すでに臨床適用されている薬剤・人工骨・加温装置を使用することで、ヒトにおいて熱刺激による骨新生促進を目指している。

股関節外科グループ
寛骨臼形成不全患者に対する偏心性寛骨臼回転骨切り術の術前予後予測スコア
当科では1989 年より寛骨臼骨切り術の一種である偏心性寛骨臼回転骨切り術を実施し、今まで良好な治療成績を報告してきた。700名以上の臨床データから本法の術後成績不良因子として発育性股関節形成不全の既往、関節適合性、BMI、術前最小関節裂隙、術前外転可動域が抽出され、これを用いた簡便な予後予測スコアを開発した。このスコアリングシステムは寛骨臼形成不全患者に対して偏心性寛骨臼回転骨切り術の適応を判断する際に有効なツールである。

股関節手術前後のQOL評価
術後のアウトカム評価は医療者側の客観的評価とともに患者側の主観的評価も重要である。股関節疾患では疼痛の軽減や歩行など日常生活動作の改善に加え、QOLの向上も期待され手術が行われる。我々は専門外来の初診患者と手術患者を対象に術前後のQOLを調査し、患者背景や治療別に治療効果を検討している。

日本整形外科学会股関節疾患評価質問表 (JHEQ)は日本股関節学会により作成され、和式生活に対する質問項目を含む疾患特異的尺度である。我々は実施や集計の手間を軽減するため、タブレット端末向けアプリケーションを開発し信頼性と妥当性を検討している。

北海道八雲町住民検診
運動器疾患の早期診断、予防や治療法の策定に資する基礎データの取得を目的として、北海道八雲町において住民の運動器疾患の検診と指導を続けている。1997年からこれまで19年間、延べ13,000名以上に調査を行った。主に脊椎疾患、膝関節疾患、筋力、骨粗鬆症、ロコモティブシンドローム、サルコペニア、QOL、栄養摂取状況を調査し、血液検査や認知機能検査も行っている。これまで得られた新たな知見は、変形性膝関節症や骨粗鬆症では10年後の死亡率がともに2.3倍高いこと、抗酸化物質であるカロテノイドが変形性膝関節症の発症予防に有効であること、変形性膝関節症の早期にはレントゲン上の関節裂隙狭小化と骨棘形成に関連性が低いことなどである。
脊椎グループ 脊髄損傷に対する多角的研究
脊髄損傷は、運動・感覚・自律神経機能の障害を引き起こし、依然として有効な治療法が限られる重要な医療課題です。脊髄損傷の根本的治療を目指し、または生活の質（QOL）を高めるために、様々な角度から研究を行っています。
Enoxaparinによる脊髄損傷治療の可能性
ヘパラン硫酸は軸索再生を妨げるシグナルを抑制します。そこで、低分子ヘパリンである既存薬Enoxaparinが軸索再生阻害を解除する作用を示し、ラット脊髄損傷モデルの機能回復を促進することを証明しました。Enoxaparinは新規神経再生治療候補であり、中枢神経損傷に対する臨床応用が期待されます。
脊髄損傷後の神経とミクログリアの相互作用
ミクログリアは中枢神経系の免疫担当細胞であり、損傷後の環境調節や神経回路の再構築に関与して回復過程を左右します。マウスモデルの脳および脊髄の生体イメージングによって脊髄損傷後の脳神経細胞の神経活動の変化や損傷軸索とミクログリアとの相互作用を見出しました。
脊髄損傷後の線維性瘢痕の病態解析
脊髄損傷では、損傷部に形成される瘢痕が神経軸索の再生を阻害します。この瘢痕はグリア瘢痕と線維性瘢痕があり、線維性瘢痕の病態は未解明です。線維芽細胞マーカーであるメフリンに着目し、脊髄損傷後の線維性瘢痕の病態の一端を明らかにしました。
