実験実習で有害物質を取り扱う学部生、大学院生及び研究生は、保健管理室のホームページ「お知らせ」から
2022年度特殊健康診断（有害化学物質取扱者）をクリックしていただき、WEB調査をお願い致します。

調査の対象者： （１）有機溶剤を取り扱う学生
　　　　　　　（２）特定化学物質を取り扱う学生
※ ただし、休学者と他大学に留学中の者は調査対象ではありません。

調査の目的： 実験・実習・研修等で有害化学物質を取り扱う学生の健康状況調査を目的とします。
なお、ＷＥＢ調査を行い、健診が必要と判断された学生については健診を１０月に行います。
WEB調査入力を行うと、対象者にはダウンロードボタンがでます。ダウンロードした調査票を印刷して、
健診当日に学生証と合わせて持参してください。

特殊健康診断（有害）は下記のとおりです。

１．検査対象者　　学生・大学院生及び研究生（ＷＥＢ調査を行い、健診が必要と判断されたもの）
２．検査日時　　　１０月１２日（水）・１３日（木）　１３：３０～１５：３０
３．検査場所　　　保健管理室

To All Students who Fill the Requirements Below

(1) Students using organic solvents (2) Students using specific chemical substances have to access the website of
Nagoya University Health Administration Office and take a survey of "Status of Hazardous Substance" on the website below.
WEB survey starts from September 6, 2022.

This survey is not applicable for students who are taking a leave of absent from school and studying abroad.

Purpose of the survey: The purpose is to investigate the health status of students who handle hazardous chemical substances in experiments, practical training, and training.
For those students who have completed the web survey and have been determined to be in need of a medical checkup,
a medical checkup will be conducted in October.
After entering the web survey, a download button will appear for those who are eligible. Please print out the downloaded survey form and bring it with you on the day of the checkup along with your student ID card.

Special medical examination will be held as follows.
