学生各位
以下の通り、第３回学生特殊健康診断（以下、ＲＩ検診）を実施します。
必ずお読みください！第3回学生特殊健康診断（放射線従事者）
放射線障害防止法第２３条により、放射線を取り扱う者は、初めて管理区域に立ち入る前にRI健診を受ける必要があります。
■対象学生■ ２０２１年１１月～２０２１年１２月に初めてX線あるいはRIの取扱いを始める学生
※ただし、職員（医員・技術補佐員）の身分を兼ねる、職員のＲＩ検診の対象者となっている学生は、 人事係の案内に従い職員RI健診を優先してください。
■必要書類 ■ 問診受験票 ※学務課大学院係窓口で配布中
■健診日程■ ２０２１年１０月１４日 （木）/ １０月１５日（金）１３：３０～１５：３０
■予約方法■ ９月６日１０時 ～ １０月１５日９時まで 名大ID/PWを使用して、予約システムへログイン。

■注意事項■
①予約日時を確認し受診してください。 ②当日は学生証、及び問診受検票（※学務課大学院係にて配布）を必ず持参してください。 ③マスクを必ず着用してください。

To Target Student
Special medical examination for students who use radioisotopes(RI) or radiation in 2021 fiscal year
Please see the details. Announcement of 3rd examination
According to the 23th article of Laws Concerning the Prevention from Radiation Hazards due to Radioisotopes and Others, users of RI or radiation must undergo the above special medical examination before they enter controlled area for the first time. Health check sheet is issued at Student Affairs Division. Please come to Student Affairs Division and get it.
■Eligible Student■ Students use radioisotopes(RI) or radiation from November 2021 to December 2021 for the first time
■Required Documents■ Medical questionnaire form *Please come to Student Affairs Office on Tsurumai Campus for picking it up
■Examination Schedule■ October 14th (Thu) to October 15th (Fri) 13:30 - 15:30
■Place■ Noyori Conference Hall
■How to reserve■ reservation starts on September 6th 10:00 - October 15th 9:00
