2020年度 学生有害化学物質取扱状況調査実施のお知らせ　　Survey on using hazardous chemical substances for students 　

学生各位

2020年度 学生有害化学物質取扱状況調査を行います。

【対象者】実験、実習・研究等で有害物質（有機溶剤、特定化学物質）を取り扱う学部生・大学院生・大学院研究生・研究生のみ 　　　　
               2019年7月から2020年7月までの1年間で、同じ物質を週１（年間50日）以上取り扱う学生です。　休学、留学者は検査対象外です。
　　　　　職員の身分を有する学生は、学生身分で有害物質の取扱いをする場合は対象となります。

今年度、該当する学生は「有害化学物質取扱状況調査票（以下、有害調査票）」を大学院係まで取りに来てください。
昨年度の有害調査票と内容が違いますので、前年の有害調査票は使用しないでください。昨年度の有害調査票は受取れませんのでご注意ください。

提出期限：７月３１日（金）までに大学院係に提出　　

今年度は、コロナウィルス感染症対策のため、10月に予定していた有害健診は中止としました。有害調査票提出により有害取扱状況や健康状態を把握し、体調不良等の学生がいましたら、個別に保健管理室で受診していただきます。

８月に実施される定期健康診断は受診してください。他機関の健康診断を受診された方、される方は、2020年1月～2020年12月に受診した健康診断結果のコピーを大学院係までご提出ください。コピーには学生番号を明記するようにしてください。　コピーの提出があっても、自動発行機および大学からの健康診断証明書の発行はできません。

今年度は、提出された調査票の返却はありません。

【Eligible Student】Students using organic solvents and specific chemical substances one day a week, and more (50days a year) from July 2019 to July 2020
*This survey is not applicable for students who are taking a leave of absent from School and studying abroad.

 Please come to the student affairs office on Tsurumai Campus  for picking up the check sheet and submit it by July 31th 2020.

Special medical examination for Hazardous Chemical Substances in October 2020 is cancelled to prevent Coronavirus Infection.  
2020 Academic Year Student Periodical Health Checkup is required. 

*If you are a School of Medicine student serving as a staff member at the University, please apply for the staff medical examination.
*For those who have had their medical examination at another institution: Please submit the copy of results of your health checkup to the student affirs office on Tsurumai Campus. Results of health checkups taken at other institutions are valid only when they are taken in between January to December of this year.  Once we receive a copy of results of your health checkup, we will recognize that you have had your health cheakup for this year.
