学生各位   To Target Student

以下の通り、第4回学生特殊健康診断（以下、ＲＩ検診）を実施します。Special medical examination for students who use radioisotopes(RI) or radiation

コロナウィルス感染防止の観点から、完全予約制にて検査を行います。Reservation is required for the examination on the website to prevent Coronavirus Infection

■対象者 Eligible Student
２０２１年１月～２０２１年５月に初めてX線あるいはRIの取扱いを始める学生
Students use radioisotopes(RI) or radiation from January 2021 to May 2021 for the first time

■必要書類 Required Documents
①問診受験票 Medical questionnaire form  ※学務課で配布中　Please come to Student Affairs Office on Tsurumai Campus for picking it up
②定期健康診断問診調査票 Health Checkup Survey　※春の定期健康診断を行っていない学生　Students do not take their medical examination in April yet

※春の定期健康診断を行っていない学生は、定期健康診断問診調査票を以下のHPよりプリントアウトし記入して、検診当日持参してください。 Students do not take their medical examination in April yet, check the below Website, print Health Survey sheet out, fill it out and bring it with you on the appointed day.

■予約方法  How to reserve  

11月中旬に保健管理室HPにて予約開始　 The reservation date is on the website of Hoken-kanri-shitsu in Higashiyama yard.　
Reservation will start in the mid of November.

■健診日程 Examination Schedule

２０２０年１２月３日 (木)、４日（金）　１３：３０～１５：３０   December 3rd (Thurs) ,4th(Fri)　 １３：３０～１５：３０        

■健診場所　Venue for Examination

東山キャンパス　保健管理室 　 Health Administration Office at Higashiyama Campus

■注意事項　NOTES

①予約日時を確認し受診してください。Please take an examination on the date reserved.
②当日は学生証、及びRI問診受検票（※学務課にて配布）を必ず持参してください。
Please make sure  to bring your student ID and the Medical questionnaire form on the date reserved.   Please come to Student Affairs Office on Tsurumai Campus for picking the Medical questionnaire form up　　　
③マスクを必ず着用してください。Please make sure to wear the mask.

放射線障害防止法第２３条により、放射線を取り扱う者は、初めて管理区域に立ち入る前にRI健診を受ける必要があります。

According to the 23th article of Laws Concerning the Prevention from Radiation Hazards due to Radioisotopes and Others, users of RI or radiation must undergo the above special medical examination before they enter controlled area for the first time. Health check sheet is issued at Student Affairs Division. Please come to Student Affairs Division and get it.

※ただし、職員（医員・技術補佐員）の身分を兼ねる、職員のＲＩ検診の対象者となっている学生は、 人事係の案内に従い職員として受診し、診断後に結果のコピーを大学院係まで提出してください。また、他機関で受診される方も健診後に結果のコピーを大学院係まで提出してください。　

※今回の健診日時に受診できない場合は、同一の健診項目について他の医療機関にて検査を受け、その結果の証明書（問診受験票）を学務課まで提出してください。
