申請依頼完了から大学開校日平日3～5日以内としてます。（土日祝日は含みません。）Please allow 3 business days for the issuance of documents for certificates in Japanese and 5 business days for certificates in English to be received at the counter or by mail.

〇本学以外で作成された様式に公印が必要な方（例）学歴照会機関、ECFMG、WESからの依頼など

オンライン申請後の払い戻し、ご返金はしておりませんので、申請時はご注意ください。Please note that we do not offer refunds or refunds after online applications have been submitted.

名古屋大学【卒業生】証明書発行について　Nagoya University【For Alumni】 Information about Certificates

〇海外からサービスを利用される方へ For those who use the service from overseas　
 海外からご利用になる場合、端末のタイムゾーンの設定により新規登録で入力をする生年月日や卒業（修了）年度の日付が前日または前月になることがあります。 その場合、生年月日や卒業（修了）年度を翌日または翌月で入れてください。
If you are using the service from overseas, the time zone setting on your device may cause your date of birth or year of graduation (completion) to appear as the previous day or month.In that case when you register your identification. In this case, enter your date of birth or year of graduation (completion) with the following day or month.

学業成績証明書について　Academic Transcript

博士課程の学業成績証明書は、原則コンビニで印刷はできません。ただし、他研究科で取得した単位がある方は、コンビニで成績証明書を印刷はできますが、医学系研究科で取得した単位の記載はありませんので、ご了承ください。

修士課程の学業成績証明書は、2023年3月卒業以降の方はコンビニで印刷可能です。それ以前の卒業者の方は、コンビニで印刷はできません。ただし、他研究科で取得した単位がある方は、コンビニで成績証明書を印刷はできますが、医学系研究科で取得した単位の記載はありませんので、ご了承ください。

In principle, doctoral program academic transcripts cannot be printed at convenience stores in Japan. However, if you have earned credits at another graduate school, you can print your transcript at a convenience store in Japan, but please note that the credits earned at the Graduate School of Medicine will not be listed.

Master's program academic transcripts can be printed at convenience stores in Japan for students graduating in March 2023 or later. Students who graduated before that date cannot print at convenience stores in Japan. However, if you have earned credits at another graduate school NU, you can print your transcript at a convenience store in Japan, but please note that the credits earned at the Graduate School of Medicine will not be listed.

単位等認定証明書・在学期間証明書について Fulfillment Certificate and Enrollment Period Certificate

単位等認定証明書（いわゆる満期退学証明書）及び在学期間証明書は、コンビニで印刷はできません。Fulfillment Certificate and Enrollment Period Certificate cannot be printed at convenience stores in Japan.

満期退学後もしくは研究生終了者が課程博士の学位取得及び論文博士の学位取得した場合の学位証明書について　Degree certificate for those who obtained a degree after completing their studies(ABD)

卒業生向け証明書サイトからオンライン申請を利用して申請をしてください。コンビニで印刷はできません。

【希望証明書の表示がない場合】
郵送希望の場合・・・「その他証明書」を選択し、次のページにある郵送オプションの備考欄に、『学位証明書※和文もしくは英文のどちらか希望を記入』と記載ください。
窓口受取希望の場合・・「その他証明書」を選択し、申請後に大学院係までメールで証明書種類のご連絡をお願いします。

論文博士学位取得者　※名古屋大学大学院での学籍を持たない方　※研究生として在籍していた方は、オンライン申請サービスをご利用ください。

研究生として在籍がある方はオンライン申請をご利用ください。新規利用登録時は、研究生の身分の情報で登録してください。
コンビニ発行できる証明書はありません。郵送・窓口受取のみ。

〇学位証明書　
　　以下の書類を添付の上、学務課大学院係(med@t.mail.nagoya-u.ac.jp)までご連絡ください。
　　●証明書交付申請用紙_Applicationformforcertificates.xlsx
　　●本人確認書類のコピー　(氏名、住所記載部分がわかるもの)　
　　●郵送先住所　（メール本文に記載ください）※窓口受取をご希望の場合は不要。
　　●論文題目が必要な方は備考欄に「論文題目希望」と記載ください。
　　●在学当時と氏名が異なる場合は、事実が確認できる書類（戸籍抄本など）を提出

　　ご申請いただいたのち、証明書代金、発行手数料等のお支払い方法をご案内致します。
　　※指定口座振込手数料はご本人様ご負担となります。
　　※証明書の発行は発行手数料（郵送料も含む）の支払が確認できた後となるため、お時間がかかる場合がございますのでご了承ください。
　　※原則として本人による申請しか受け付けられません。

注意事項　NOTE

• 現在本学在学中で過去に在籍していた課程に係る証明書の発行については、システム上は卒業者・修了者と同じ扱いとなりますが、学内に設置されている証明書自動発行機からは無料で出力できます。
• 厳封について　Seal the certificate
  コンビニエンスストアで発行した証明書を厳封することはできません。厳封を希望する場合は，窓口受け取りまたは郵送申請をしてください。
  If you wish to seal the certificate, please select by mail or picking up at University counter.
• 在学当時と氏名が異なる場合
  申請書には在学時の氏名を記入し、現在の氏名を併記してください。
  その上で事実が確認できる書類（戸籍抄本等）を用意し、申請の際に提示又は提出してください。
  証明書は、在学当時の姓名でしか作成できません。　必要に応じて、作成した証明書に戸籍抄本を添付する等してご使用ください。
