High-Cost Medical Care Benefit System/Gendogaku Tekiyo Ninteisho (Certificate of Eligibility for Reduced Co-Payments)

Under High-Cost Medical Care Benefit System, you can reduce your monthly medical care cost according to your income. Patients under 70 years old are only required to pay their medical cost up to the maximum copayment amount (excluding meal expenses and extra charges) by showing the certificate "Gendogaku Tekiyo Ninteisho" to the cashier. You should apply to your health insurer in advance for the certificate.

"Gendogaku Tekiyo Ninteisho" is applied from the month when it is presented, so please bring it to the hospitalization reception desk on the first floor in Ward Building or to the cashier on the first floor in Outpatient Building as soon as you it is delivered.
For details about the application for "Gendogaku Tekiyo Ninteisho", please ask your own insurer directly.

※ When you come into hospital at the end of the month and apply for the certificate to your insurer in the next month, the reduction might not be applied to the first month.
※ If you are outpatient, please show the certificate at the first visit of each month.

The maximum copayment amount has 3 stages according to your income as below. (Jan.1.2014)

"Multiple application" amount is the maximum amount applicable from the fourth payment when one household makes over four payments towards high cost medical care within 12 months.

To receive reimbursement of high-cost medical care benefit

Patients who have made payment without "Gendogaku Tekiyo Ninteisho" can apply for a reimbursement to your insurer after your payment.

【Those insured under the National Health Insurance system】

If you are eligible for reimbursement of high-cost medical care benefit, you will receive a postcard in the mail notifying you of your reimbursement from the municipal office about 2-3 months after receiving the treatment. With the postcard, you can claim the reimbursement at the municipal office. (For more detailed information, please contact your local municipal office.)

【Those insured under an employees' health insurance (social health insurance) system】

  • Japan Health Insurance Association (JHIA) -managed insurance: For information, please contact the Japan Health Insurance Association branch shown at the your health insurance card.
  • Kyosai Hoken Kumiai or Mutual Aid Association (MAA)-managed insurance: For information, please contact the department in charge of your company's mutual aid association.
  • Kenko Hoken Kumiai or Association-Managed Health Insurance (AMHI): For information, please contact the department in charge of your company's managed health insurance.
