What to bring to the Outpatient Clinic

First-time patients

Patients whose regular hospital or clinic has faxed an application for examination
  • Health Insurance Card (or Certificate of Eligibility for Insurance), any certificates for medical expenses deductions
  • Application form for consultation at Nagoya University Hospital (the form faxed by your regular hospital or clinic)
  • Letter of referral (Patient Referral Document)
  • Image data(CD or DVD), etc. (if applicable)
Other first-time patients
  • Health Insurance Card (or Certificate of Eligibility for Insurance), any certificates for medical expenses deductions
  • Letter of referral from another hospital or clinic (see note below)
  • Image data(CD or DVD), etc. (if applicable)

Note: Please be aware that patients without a letter of referral are required to pay an additional fee of ¥4,320 as an extra cost not covered by health insurance.

Repeat patients

  • Hospital Card
  • Health Insurance Card, medical certificates, etc. (these will be checked at your first examination each month)

Monthly submission of Health Insurance Card and medical certificates

Please submit your Health Insurance Card and any certificate for medical expenses deductions to the hospital for checking once a month.
After your first examination of the month, please submit them to the Cashier (Receipts) together with your reception slip. Please also tell the Cashier (Receipts) if there are any other changes to your registered information, such as change of address or telephone number.
If your Health Insurance Card cannot be checked you may not use your health insurance and must initially pay the full amount of the bill yourself.
In that case, you can either bring your Health Insurance Card to the hospital at a later date and receive a refund of the portion of your bill covered by insurance (only permitted within the same month), or apply to your insurance provider for a refund.

Please be sure not to forget your Health Insurance Card.
