Information for First-Time Patients

First-time patient reception

Reception hours for first-time patients are from 8:30 to 11:00.

Note: Patients who have previously been examined in a different department at Nagoya University Hospital but are undergoing their first examination in a new department, and patients whose last previous examination at this hospital was more than five years ago, must complete first-time patient reception procedures.

In principle, outpatient examinations at Nagoya University Hospital are performed according to an appointment system.

If you come to the hospital without having made an appointment, your waiting time will be longer and you may be asked to make an appointment for another day.

Please make an appointment in advance. See the page on Outpatient Appointments for information on how to make an appointment.

First-time patients who have an appointment (whose regular hospital or clinic has faxed an application for examination in advance)

Please submit the following documents to the Patient Registration on the first floor of the Outpatient Building by 30 minutes before your appointment time:

  • Health Insurance Card, any certificates for medical expenses deductions
  • Application form for consultation at Nagoya University Hospital (the form faxed by your r egular hospital or clinic)
  • Letter of referral (Patient Referral Document)
  • Image Data( CD or DVD ), etc. (if applicable)

In this case, you do not need to fill in an Application form for consultation at the desk or take a numbered ticket.

Patient Registration will issue you a Hospital Card and any other necessary items. Please submit these to the reception desk in the clinical department in which you will be examined by 10 minutes before your appointment time.

Other first-time patients

During first-time patient reception hours, take a numbered ticket from the dispenser in front of the Patient Registration and fill in the Application form for consultation.
Wait for your number to be called

When your number is called, please submit the following items to the Patient Registration:

  • Numbered ticket
  • Application form for consultation
  • Health Insurance Card, any certificates for medical expenses deductions
  • Letter of referral from another hospital or clinic, Image Data( CD or DVD ), etc. (if applicable)

The Patient Registration will issue you a Hospital Card and any other necessary items. Please submit these to the reception desk of the clinical department in which you will be examined.

Note: Please be aware that patients without a letter of referral are required to pay an additional fee of ¥5,400 as an extra cost not covered by health insurance.

Patients who already have a Hospital Card but are being examined in a new clinical department

During first-time patient reception hours, take a numbered ticket from the dispenser and wait for your number to be called. (You do not need to fill out an Application form for consultation.)

Alternatively, if you have been issued a "Guide to Examination by a Different Clinical Department" by a doctor at this hospital, please submit this directly to the reception desk of the clinical department in which you will be examined.

When your number is called, please submit the following items to the Patient Registration :

  • Numbered ticket
  • Health Insurance Card, any certificates for medical expenses deductions
  • Hospital Card

The Patient Registration will issue you Reception Card and any other necessary items. Please submit these to the reception desk of the clinical department in which you will be examined.

Note 1: Patients being examined under the Public Expense or Workers Compensation systems should fill out an Application form for consultation and go to the Public Funded Medical Service Reception.
Note 2: 24-hour emergency outpatient treatment is available for patients with urgent symptoms.
