Procedures for Hospitalization

Prior to hospitalization

Necessary documents
  1. Hospitalization Request Form: Please read carefully the Hospitalization Request Form provided at the Hospital Admission Center or the clinical department concerned , and answer all the questions that apply to you. Bring the completed form to the hospital on the day of your arrival. If you have a hospital discharge certificate from another hospital, please submit it with the Hospitalization Request Form.
  2. Confirmation Form for Rental Patient Clothes
  3. Health Insurance Card (Your health insurance card, certificate for medical expenses deduction and/or other insurance-related documents, which are used to confirm your eligibility, will be returned immediately after this has been confirmed.) If any change has been made to the information on your health insurance card or you have lost your eligibility for the health insurance plan, please notify the Inpatient Reception immediately.
  4. Hospital Card
  5. If you have other certificates such as a Fukushi Techo (Mental Health and Welfare Notebook) that may be needed for hospitalization, please present them, too.
Procedures
  1. On the day of your arrival, please submit the completed Hospitalization Request Form, Confirmation Form for Rental Patient Clothes, Health Insurance Card, Hospital Card, and Fukushi Techo or other certificates, if you have them, to the Inpatient Reception (first floor of the Ward Building) and start the hospital admission procedures by around 10:00 a.m. (If you delay submitting your insurance card and other necessary documents, you may have to pay the entire cost yourself until you submit them.)
  2. After completing the required procedures at the Inpatient Reception, you will be provided with a file of documents, including the Patient Registration Card. Please go to the Nurse Station of the ward where you will be admitted and give the file to a nurse.
  3. Those who have reserved a special room in advance are required to sign and submit the "Agreement for Use of a Special Room" at the Inpatient Reception.
Remarks
  1. The parking lot of the hospital is exclusively for outpatients and not for inpatients' use.
  2. To park a car to transport an inpatient, please go to the Inpatient Reception to punch the parking ticket (during non-office hours and on holidays, at the After-Hours Reception Desk or the General Information). This does not apply to people visiting an inpatient.
What to bring for your hospital stay

Minimum personal care items, including underwear, toiletries (toothbrush, toothpaste, soap, shampoo, a basin for washing, etc.), bath towel, washcloth, thermos bottle, teacup, chopsticks, spoon, facial tissues, earphones, and non-slip room slippers

For hospitalization for childbirth or surgery, please ask the clinical department where you are treated about what to bring in addition to the items listed above.

Note

To avoid theft, do not bring a large amount of cash and/or valuables.
Lockers are available in the wards for storing your wallet and valuables. Please ensure that they are kept locked during use.
For your convenience, a post office is located on the first floor of the Ward Building and there is a Bank of Tokyo-Mitsubishi UFJ ATM on the first floor of the Outpatient Building.
