運動・形態外科学整形外科学／リウマチ学

研究室概要

整形外科学は運動器を構成する骨軟骨、筋腱、靱帯、脊髄および末梢神経などの先天性・後天性の疾病や外傷を診療の対象とし、保存治療と外科治療を組み合わせながらその障害の克服を目指す専門領域です。近年超高齢社会に突入した本邦では、健康寿命の延伸に向け運動器障害、いわゆるロコモティブシンドロームの病態解明と治療法の開発が喫緊の課題となっており、その中心的な役割を担う整形外科学分野の重要性が増しています。当研究室は脊椎外科、股関節外科、関節リウマチ外科、小児整形外科、膝肩スポーツ外科、骨軟部腫瘍外科の6つの診療グループから構成されています。各グループは相互に連携しながら日々の診療に携わり、またグループごとに治療手技の創意工夫や開発に努めています。昨今の分子遺伝学や分子生物学、再生医療の長足の進歩によって、今まではアプローチできなかった先天性および変性疾患の病態解明や脊髄損傷、軟骨変性、骨系統疾患などの根本的治療法の実現が夢ではなくなっています。当教室でもこれら難治性運動器疾患の克服に向けたトランスレーショナルリサーチを基礎医学系教室と連携しながら鋭意進めています。

研究プロジェクト

小児班

小児班では軟骨無形成症の新規治療法開発研究を行っています。軟骨無形成症は四肢短縮型低身長を示す指定難病で、病因は骨伸長の抑制因子であるFGFR3の恒常的活性化です。低身長に対する従来の治療法は成長ホルモン注射と骨延長手術ですが、前者は効果が限定的で後者は治療期間が長く合併症が多いのが難点です。2022年8月に本邦で承認されたCNPアナログ（ボソリチド）毎日注射製剤はFGFR3抑制作用が証明されていますが、小児患者にとって毎日の注射は負担が大きいです。

内服薬メクリジンは軟骨無形成症実験モデルに有効性を示した

既存薬のスクリーニングにより乗り物酔い止め薬として使用実績のあるメクリジンにFGFR3抑制効果を見出しました。軟骨無形成症細胞モデルにおいて、メクリジンは軟骨細胞増殖抑制をレスキューしました。また、胎児マウスの脛骨にFGF2を添加したモデルにおいては、メクリジンは骨伸長を促進しました。軟骨無形成症マウスモデルにメクリジンを定量的に経口投与すると、骨伸長促進効果が認められました。さらに、メクリジンを成長期に長期間投与すると軟骨無形成症の重篤な合併症である脊柱管狭窄を一部レスキューしました。

小児軟骨無形成症を対象とした医師主導治験

メクリジンは内服薬であり小児に対する長期投与のコンプライアンスが期待できます。また、安価な薬剤による難病治療法の開発は医療経済的に有益です。PMDAと相談して治験実施に必要な前臨床試験を行ったうえで、これまでに3つの医師主導治験を実施しました。メクリジン単回投与第Ia相試験および14日間連日投与第Ib相試験で安全性に懸念がないことを確認できましたので、26週間連日投与第II相試験を実施しました。身長の増加速度に有意な改善は認められませんでしたが、アームスパンの増加速度は大幅に改善したことが示唆されました。現在次の臨床試験を検討しています。

難病プラットフォームや患者会との連携

難病である軟骨無形成症の患者さんの治療情報、身長や生活の質などを持続的・長期的に収集し、どのような治療法がよいか検討することを目的として、全国で統一した登録システムである難病プラットフォームのレジストリが構築されました。難治性疾患政策研究班と協力して研究を進めていますが、将来的にはその時点での最良の治療や臨床試験を患者さんに提案するために利活用したいと思います。患者会にも難病プラットフォームやメクリジン研究の進捗について情報提供しています。

リウマチ班

細胞内代謝変動の制御による変形性関節症の新規治療開発研究

細胞内代謝制御を標的とした変形性関節症の新規治療法開発を目指し、基礎研究を進めています。炎症環境下における軟骨細胞では、嫌気性解糖系代謝（glycolysis）の亢進をはじめとする複数の細胞内代謝変動が生じていることを明らかにしました。これらの代謝変動を統合的に制御する中心的分子として AMP-activated protein kinase（AMPK） に着目し、AMPK活性の低下が軟骨細胞に多面的な機能障害をもたらすことを示しました。さらに、グルコース代謝阻害剤 2-deoxy-D-glucose（2DG）、脂肪酸代謝制御剤 fatostatin、およびグルタミン代謝阻害剤 epigallocatechin gallate（EGCG）を用いた解析を行った結果、これらの代謝制御薬はいずれも AMPK 活性化維持し、変形性関節症モデルマウスにおける軟骨変性を有意に抑制しました。

Tsurumai Biologics Communication Registry (TBCR)

Tsurumai Biologics Communication Registry（TBCR）は、関節リウマチにおける分子標的薬使用症例のデータベース化を目的として、2008年に名古屋大学が中心となりリウマチ診療関連施設との連携のもとに構築されたレジストリーです。各症例を縦断的に調査する国内有数の大規模レジストリーであり、登録症例数は4,000例を超えています。臨床試験では得ることが困難な実臨床に即したデータや、市販後調査では十分に把握できない長期にわたる治療成績および安全性に関する情報を蓄積・解析してきました。本レジストリーから得られた知見は、関節リウマチ診療における分子標的薬治療の最適化に寄与しています。

特定臨床研究

名古屋大学医学部附属病院は、2016年1月に医療法に基づく臨床研究中核病院として承認されています。臨床研究中核病院には、特定臨床研究を実施し、その成果として質の高い論文を発表することが求められています。当グループでは、これまでに2つの特定臨床研究を完遂し、その成果として3編の論文を公表してきました。これらの研究を通じて、科学的妥当性および倫理性を重視した臨床研究を推進し、関節リウマチ診療におけるエビデンス創出に取り組んでいます。

膝肩スポーツ班

変形性膝関節症を抑制する因子の探索

変形性膝関節症は、膝関節軟骨の変性や骨棘形成を背景に疼痛を生じ、日常生活動作を制限して生活の質（QOL）を低下させる疾患です。現在、臨床ではヒアルロン酸の関節内注射が広く用いられていますが、効果を維持するために複数回の注射が必要となる場合があります。私たちの研究班は、こうした現状を踏まえ、ヒアルロン酸関節内注射に代わり得る薬物治療の開発を目指して探索研究を進めています。当班はヒアルロン酸合成酵素(HAS2)の軟骨細胞への過剰発現がウシ・ヒト軟骨細胞において抗炎症作用を及ぼすことを見出しました。現在はウイルスベクターの関節内注射によるHAS2の過剰発現が軟骨組織に対して抗炎症作用を及ぼすかどうかについて重点的に研究を進めています。

腱板断裂に対する薬物治療の探索

腱板断裂は外傷によって生じる事もありますが、ほとんど場合は加齢や炎症に伴う変性によって生じ、反復するストレスや血流の低下、マトリックス組成変化などの内因性因子が関連していると考えられています。症候性腱板断裂に対しては、臨床現場でヒアルロン酸、ステロイド、局所麻酔薬（キシロカインなど）の関節内注射が行われることがあります。しかし、腱板断裂そのものを適応として保険診療で使用できる有効な薬物療法は、現時点で我が国には確立されていません。当班では、前十字靭帯由来細胞においてヒアルロン酸合成阻害剤である4-メチルウンベリフェロン(4-MU)の抗炎症効果を見出しました。靭帯と腱は組織学的に類似しているため、腱由来の細胞においても4-MUの抗炎症効果が期待でき、腱板断裂の新規薬物治療になりうると考え、研究を行っています。

腫瘍班

骨・軟部腫瘍は希少疾患である一方、生命予後のみならず、治療後の運動機能や生活の質（QOL）に大きな影響を及ぼします。腫瘍班では、腫瘍の病態解明から治療成績・機能温存の向上までを一体として捉えた研究を推進しています。基礎研究、トランスレーショナルリサーチ、臨床研究を有機的に結びつけることで、次世代の腫瘍治療の確立を目指し、多角的なアプローチで研究に取り組んでいます。

温熱刺激による骨形成促進に関する研究

骨・軟部腫瘍の再建手術後において、骨形成や骨癒合の遅延は機能回復を妨げる重要な課題です。我々は、温熱刺激が骨代謝および骨形成に及ぼす影響に着目し、基礎実験および動物モデルを用いて研究を行っています。
温度刺激が骨芽細胞の活性化や骨形成シグナルに与える影響を解析することで、侵襲の少ない新たな骨形成促進法の開発を目指しています。

Next Generation Sequencing（NGS）を用いた未解明腫瘍の病態解明

骨・軟部腫瘍には稀な疾患が多く、病理学的な特徴や病態が未解明の腫瘍が存在しています。そのような腫瘍の分子病態を明らかにすることを目的とし、学内および多施設共同でNext Generation Sequencing（NGS）を用いた遺伝子解析を行っています。 遺伝子変異や融合遺伝子、コピー数異常、シグナル経路の異常などを同定することで、診断精度の向上や新規治療標的の同定を目指します。

新規人工関節の治療成績と骨の力学試験

悪性骨腫瘍に対する四肢温存手術では、人工関節再建が重要な役割を果たしますが、長期成績においてはインプラント周囲骨の強度や骨形成が課題となります。我々は、新規人工関節システムと骨強度・骨形成との関連に注目し、画像評価および臨床データを用いた解析を行っています。骨質や力学的負荷とインプラント安定性の関係を明らかにすることで、より耐久性が高く、機能温存に優れた再建法の確立を目指しています。

股関節班

寛骨臼形成不全患者に対する偏心性寛骨臼回転骨切り術の術前予後予測スコア

当科では1989 年より寛骨臼骨切り術の一種である偏心性寛骨臼回転骨切り術を実施し、今まで良好な治療成績を報告してきました。700名以上の臨床データから本法の術後成績不良因子として発育性股関節形成不全の既往、関節適合性、BMI、術前最小関節裂隙、術前外転可動域が抽出され、これを用いた簡便な予後予測スコアを開発しました。このスコアリングシステムは寛骨臼形成不全患者に対して偏心性寛骨臼回転骨切り術の適応を判断する際に有効なツールです。

股関節手術前後のQOL評価

術後のアウトカム評価は医療者側の客観的評価とともに患者側の主観的評価も重要です。股関節疾患では疼痛の軽減や歩行など日常生活動作の改善に加え、QOLの向上も期待され手術が行われます。我々は専門外来の初診患者と手術患者を対象に術前後のQOLを調査し、患者背景や治療別に治療効果を検討しています。

タブレット版日本整形外科学会股関節疾患評価質問表

日本整形外科学会股関節疾患評価質問表 (JHEQ)は日本股関節学会により作成され、和式生活に対する質問項目を含む疾患特異的尺度です。我々は実施や集計の手間を軽減するため、タブレット端末向けアプリケーションを開発し信頼性と妥当性を検討しています。

脊椎班

脊椎班では脊髄損傷に対する多角的研究を行っています。脊髄損傷は、運動・感覚・自律神経機能の障害を引き起こし、依然として有効な治療法が限られる重要な医療課題です。脊髄損傷の根本的治療を目指し、または生活の質（QOL）を高めるために、様々な角度から研究を行っています。

Enoxaparinによる脊髄損傷治療の可能性

ヘパラン硫酸は軸索再生を妨げるシグナルを抑制します。そこで、低分子ヘパリンである既存薬Enoxaparinが軸索再生阻害を解除する作用を示し、ラット脊髄損傷モデルの機能回復を促進することを証明しました。Enoxaparinは新規神経再生治療候補であり、中枢神経損傷に対する臨床応用が期待されます。

脊髄損傷後の神経とミクログリアの相互作用

ミクログリアは中枢神経系の免疫担当細胞であり、損傷後の環境調節や神経回路の再構築に関与して回復過程を左右します。マウスモデルの脳および脊髄の生体イメージングによって脊髄損傷後の脳神経細胞の神経活動の変化や損傷軸索とミクログリアとの相互作用を見出しました。

脊髄損傷後の線維性瘢痕の病態解析

脊髄損傷では、損傷部に形成される瘢痕が神経軸索の再生を阻害します。この瘢痕はグリア瘢痕と線維性瘢痕があり、線維性瘢痕の病態は未解明です。線維芽細胞マーカーであるメフリンに着目し、脊髄損傷後の線維性瘢痕の病態の一端を明らかにしました。
