平成31年度　定期健康診断のお知らせ（WEB予約をしてください）
2019.4-2020.3　Annual Health Checkup for Students in Higashiyama (WEB reservation is necessary)

平成31年度定期健康診断を行います。
必ずWEB予約をしてください。予約には名大ID・パスワードが必要です。
尿検査容器は大学院係まで取りに来てください。

Annual Health Checkup for Students is held in March and April.
Please make a web reservation in advance.
Nagoya University ID and password are required for a web reservation.
Please pick up the urine sample container at Student Affairs Division.

◇3月定期健康診断
対象者：医学系研究科大学院生（M1・D1・D2・D3）
※平成30年度現在の学年です。
※平成31年度にD1に進学する学生は4月の指定日に受診してください。

Applicants: Master Course 1st grade　Doctoral Course 1st, 2nd, 3rd grade
※AS of AY 2018
※The students who will enter Doctoral Course in April need to take a health checkup in April.

対象者：医学系研究科大学院生、研究生、大学院研究生
Applicants: Graduate Student(MC, DC), Research Student, Graduate Research Student

AY2019 April Student health checkup

※ただし、本学の教職員（医員を含む）の身分を持つ者は、平成３１年度の職員健康診断を受診してください。職員健康診断または他の医療機関で健康診断を受診された場合は、当該健康診断結果（受診日が平成３１年１月～１２月のものに限る）の写しに学生番号を付記し、学務課大学院係へ提出してください。健康診断書写しの提出により、今年度の健康診断を受診したものとみなします。
