有害化学物質取扱状況調査はWEB入力になります。
実験実習で有害物質を取り扱う学部生、大学院生及び研究生は、保健管理室のホームページ「お知らせ」から
2023年度特殊健康診断（有害化学物質取扱者）をクリックしていただき、WEB調査をお願い致します。
WEB調査開始日：２０２３年９月１３日　９：００～

なお、ＷＥＢ調査を行い、健診が必要と判断された学生については健診を１０月に行います。
WEB調査入力を行うと、対象となる学生にはダウンロードボタンがでます。ダウンロードした調査票を印刷して、受検予約してください。
健診当日に学生証と合わせて持参してください。

特殊健康診断（有害）は下記のとおりです。

１．検査対象者　　学生・大学院生及び研究生（ＷＥＢ調査を行い、健診が必要と判断されたもの）
２．検査日時　　　１０月１１日（水）・１２日（木）　１３：３０～１５：３０
３．検査場所　　　保健管理室

Nagoya University Health Administration Office and take a survey of "Status of Hazardous Substance" on the website below.

For those students who have completed the web survey and have been determined to be in need of a medical checkup, a medical checkup will be conducted in October.
After entering the web survey, a download button will appear for those who are eligible. Please print out the downloaded survey form and bring it with you on the day of the checkup along with your student ID card.

Special medical examination will be held as follows.

Date: October 11 (Wed) , October 12(Thu)　13：30- 15 : 30
Place: Health Administration Office at Higashiyama Campus of Nagoya University
