Certificates will be issued within 3 to 5 school days after the application is completed (excluding weekends and holidays).

If you cannot find the certificate you want on the insurance service
Please select "Other Certificate" and email the Student Affairs Division to let us know which certificate you would like. If there is a fee for issuing the certificate, it will not be refunded after application, so please contact the Student Affairs Division in advance.
Academic Transcript
Doctoral students
The method of credit recognition for students in the Graduate School of Medicine's doctoral program is to certify grades throughout their enrollment. Therefore, at this time, we are unable to issue transcripts. Instead, we issue a "Certificate of No Grades are available".
Please select "Certificate of No Grades are available" as the certificate type on the online certificate service.
Credits earned at other graduate schools will be listed on the transcript, so while grades from the Graduate School of Medicine will not be listed, we can issue transcripts for credits earned at other graduate schools.
Master's students
Grades are certified at the end of each student's academic year (March for students enrolling in April, September for students enrolling in October).
After certification, grades can be issued from an automatic issuing machine or printed at a convenience store by applying online certificate service.
We issue a "Certificate of No Grades are available" during the first year of enrollment. Please select "Certificate of No Grades are available" as the certificate type on the online certificate service.
*Grades will be certified at the end of each student's academic year, but please note that due to the certification procedures, grades for classes taken at the end of the semester, around February to March, or around August to September, will not be reflected in your grades.
*Grades are certified for each class, so for example, Basic Training will not be certified if you have taken fewer than four courses.

Documents for enrolling in a nursery school
The "Certificate of Attendance (Form No. 7)", a document required for enrolling in a nursery school or other facility, can be obtained from your local municipal office. The Student Affairs Division does not keep track of each student's attendance days and hours, and is therefore unable to prepare the document. Please ask your laboratory to prepare it for you.
For those living in Nagoya City
Please ask the head of your laboratory to prepare the Certificate of Attendance (Form No. 7). (The ward office has given permission for the head of laboratories to certify the Certificate of Attendance instead of the president.)
Please submit the certificate along with the enrollment certificate issued by the on-campus issuing machine to your local municipal office.
Regarding the issuance of a degree certificate for those who are former doctoral students conducted ABD or research students have obtained a doctoral degree.
Please apply using the Certificate Issuance Service for alumni.
If you cannot find the certificate you want on the insurance service
If you wish to have your certificate sent by mail, select "Other Certificates" and write "Degree Certificate (Japanese or English)" in the notes section of the mailing option on the next page.
If you wish to collect the certificate at the office, select "Other Certificates" and please email the Student Affairs Division to inform them of the type of certificate after applying.
