Introduction

This division provides medical care mainly for tumors in the gastrointestinal system including esophagus, stomach, duodenum, small intestine, and large intestine as well as liver, biliary tract (gallbladder and bile duct), and pancreas. We perform an aggressive surgery for highly advanced hepato-biliary pancreas, esophagus, and colorectal cancers. Especially, our department has the best treatment results for hilar cholangiocarcinoma regardless in or outside of Japan. We also perform minimally invasive surgery for selected patients in esophagus, stomach, colorectal, and hepato-biliary pancreas diseases. Through these experiences as a high volume center, our surgeons continually sharpen their skills and therefore can provide better outcomes for patients.

Research Projects

(1) Multicenter study for understanding the clinical pathology of distal cholangiocarcinoma

The clinical pathology of cholangiocarcinoma developing in the distal bile duct has not been clarified yet, largely due to the small number of the cases. We are accumulating high-quality retrospective data to understand the pathology of distal cholangiocarcinoma. Our goal is to provide the valuable information worldwide.
We have been accomplished (1) to establish the new classification system of lymph node metastasis, (2) to evaluate the preoperative approach for biliary drainage and (3) to clarify the type of recurrence and risk factors. We are now trying (4) to find out the benefit of resection and reconstruction of the portal vein, (5) to establish new tumor classification based on the depth of the invasion and (6) to evaluate the benefit of minimally invasive surgery for distal cholangiocarcinoma.

The survival is worse in PTBD (percutaneous transhepatic biliary drainage) patients than EBD (endoscopic biliary drainage) patients, due to the peritoneal dissemination caused by PTBD.

(2) Strategy to prevent the liver ischemia/reperfusion injury

The liver is an organ with rich blood flow. During liver resection, the occlusion of blood flow to the liver is a mandatory procedure to minimize the intraoperative blood loss. However, this technique at the same time produces an ischemia/reperfusion injury in the liver. We have been performing several basic experiments to develop a way to minimize the ischemia/reperfusion liver injury. The lower images demonstrate hepatic microcirculation observed by an intravital microscopy. The microcirculatory disturbance, which is normally observed following ischemic stress, was significantly attenuated by a preoperative administration of branched chain amino acids (BCAA). We also found that the mechanism of this preventive effect is at least partly mediated by the suppression of Kupffer cell activation.

Our laboratory also performing basic researches using several animal models such as 70%/90% hepatectomy model, portal branch ligation model, bile duct ligation model, partial/total cholangitis model, and septic shock model. Our goal in basic research is to provide a result that is directly translational to the clinical setting.

(3) The appropriate administration of postoperative antibiotic for the patients undergoing hepatectomy with biliary reconstruction- prospective study.

Although hepatectomy with biliary reconstruction is the invasive surgical procedure with high rate of infectious complications, there has been no evidence to indicate the appropriate administration of postoperative antibiotics for these patients. We randomized eighty-six patients into two group, one with two-day treatment and the other with 4-day. The primary endopoint was SIRS (systemic inflammatory response syndrome) on postoperative day 5, and secondary endopoints were SSI (surgical site infection) and other postoperative infectious complications. As a result, there was no significant difference in patients’ background, postoperative infectious complications and SIRS. This result suggest that 2-days postoperative antibiotic treatment is enough for the patients undergoing hepatectomy with biliary reconstruction. We will expand this study for the other surgical procedures.

(4) Multidisciplinary treatment for the locally advanced and recurrent rectal cancer
The standard treatment for the locally advanced rectal cancer is preoperative chemoradiation and TME (total mesorectal excision) in the western world. This has been reported to result in improved local control bot not in improved survival rate.
To achieve better survival rate, it is important to control rectal cancer not only locally but also systemically. We are challenging this issue by inducing preoperative chemotherapy without radiotherapy for the patients with locally advanced rectal cancer with high risk. Also, we actively perform advanced surgery including total pelvic exenteration (TPE) for locally recurrent rectal cancer and accumulating the cases to evaluate the benefit of these treatment.
We believe that extended surgery is beneficial for local control and preoperative chemotherapy for systemic control of locally advanced rectal cancer.

(5) A prospective randomized controlled trial comparing posterior mediastinal route and retrosternal route with gastric tube reconstruction after subtotal esophagectomy

The most popular reconstruction route after subtotal esophagectomy is the posterior mediastinal route in western world. While the retrosternal route is predominant in Japan, the selection of the reconstruction depends on each institute. No study has clarified the benefit of each route in detail.
In our institute, the clinical study is now ongoing to compare the benefit of these two route, regarding postoperative complications, functional outcomes and postoperative QOL (quality of life). Our goal is to determine which is the better approach to reconstruct alimentary tract after subtotal esophagectomy.

(6) Development of navigation surgery for hepatectomy

Methods
Virtual liver and body images are reconstructed using “New-VES” system developed by Nagoya University Graduate School of Information Science - Mori’s Office.

<Two-dimensional virtual navigation for laparoscopic hepatectomy>
Virtual liver and body images correspond to maps of car navigation system. Some of patient’s body parts are registered in virtual 3D liver and body images using a magnetic position sensor. Patient’s body after registration corresponds to The Earth. A transmitter for magnetic position sensor, which corresponds to an artificial satellite, is placed about 30cm above patient’s body. A micro magnetic sensor, which corresponds to GPS antenna, fixes on the handling part of laparoscope. Laparoscopic hepatectomy is performed using both real operative and virtual monitors.

It is difficult in open laparotomy to share the view like a laparoscope surgery, because the views of the operator and assistants are different. Three-dimensional liver model is manufactured by a high-quality three-dimensional printer according to virtual liver images. We utilize this three-dimensional liver model during hepatectomy.

We are developing navigation surgery to improve surgical safety in both laparoscopic and open hepatectomies.

(7) The role of Trefoil Factor Family 1 (TFF1) as tumor suppressor in pancreatic and hepatobiliary carcinogenesis.

While TFF is the proteins important for the recovery of injured gastrointestinal mucosa, the tumor suppressive role of TFF has been reported. The premalignant lesions of pancreas such as PanIN (pancreatic intraepithelial Neoplasms) and IPMN (intraductal papillary mucinous neoplasms) express abundant TFF, suggesting that TFF works as tumor suppressor in pancreas as well. We found that TFF actually prevent pancreatic carcinogenesis using genetically-engineered mice. Our goal is to clarify the mechanisms of how TFF can prevent pancreatic tumorigenesis, to invent novel strategy for pancreatic cancer and chemoprevention for pancreatic and hepatobiliary malignancy.

(8) Nucleatic acid medicine for multiple human malignancies

Nek2 (NIMA-related kinase 2) is a member of the serine/threonine kinase family, related to the essential mitotic regulator NIMA. Nek2 is involved in cell division and mitotic regulation by centrosome splitting. Nek2 was highly expressed in various cancer cell lines, such as cholangiocarcinoma, pancreas cancer, breast cancer and colorectal cancer. We reported that Nek2 suppression with siRNA resulted in the inhibition of cell proliferation and induced cell death in these cancer cell lines. Nek2 siRNA suppressed the tumor growth in xenograft cancer models. In peritoneal dissemination model, Nek2 siRNA-treated mice showed statistically longer survival periods in comparison with those of the control siRNA-treated mice.To evaluate the safety and preliminary efficacy of three doses of Nek2 siRNA, we started the translational research, entitled “Analysis of novel treatment using Nek2 siRNA for non-curative pancreas cancer” from June 2013. This is the first-in-human trial concerning Nek2 siRNA. Further investigation is required to conclude the effectiveness of Nek2 siRNA for the improvement of the prognosis of cancer patients. Nek2 siRNA may be a novel cancer treatment, holding the potential to provide regression of multiple human malignancies.

(9) A novel treatment using α-bisabolol for cancers

α-Bisabolol is a sesquiterpene alcohol present in essential oils derived from a variety of plants. This compound has several functions, such as being gastric protective, antimicrobial, and anti-inflammatory. We reported that α-bisabolol inhibits cell proliferation and induces apoptosis in pancreatic cancer cell lines. This compound also suppresses the invasiveness and motility of a pancreatic cancer cell line. α-Bisabolol suppresses tumor growth in pancreas cancer xenograft model. Early growth response 1 (EGR1) is involved in the antiproliferative effects. Further investigations are necessary to clarify the precise mechanisms of α-bisabolol activity for clinical application as a novel treatment for pancreatic cancer. α-Bisabolol has a therapeutic potential for the pancreatic cancer.

(10) A novel therapy using adipose-derived stem cell sheets for peripoperative complications

Adipose-derived stem cells (ADSCs) are multipotent cells that originate from adipose tissue. ADSCs have the ability to differentiate and regenerate. We previously reported that the intravenous administration of ADSCs enhanced liver regeneration after a hepatectomy. We have created a multilayered ADSC cell sheet using a novel magnetite tissue engineering method, and we have applied this sheet for the treatment of pancreatic fistulas. Macroscopically, more severe degeneration and adhesion in the peritoneal cavity were observed in the untreated rats than in the rats treated with ADSC sheets. Significant differences were found in both the ascitic and serum levels of amylase and lipase between the untreated rats and the rats treated with ADSC sheets. ADSC sheets are effective for preventing pancreatic fistulas and have promising potential for clinical applications.

Call for Graduate Students

Any students who are interested in physiological or oncological research for liver, biliary tract, pancreas, and other gastrointestinal system are welcome. We accept two years of master course students and four years of doctor course students. We hope that all students master scientific skill through our translational research.

Examinations for graduate course;

The master course starts in April. Applicants take written examinations of biology and English at the end of August. The written examinations are given in Japanese. Alternatively, applicants may submit recommendation letters and take a viva-voce examination. The capacity of the master course is 20 students per year in our Graduate School of Medicine, and about a half and a third of applicants are usually eligible to enter the course. In 2011, we start a G30 program for master course students, in which the course start in October and all the entrance and education processes are given in English.
The doctor course starts in April and October. The April course is for domestic and international students. The October course is only for international students. In both courses, applicants take written examinations either at the end of January or at the end of September. The examinations are given in English in either start. There is no recommendation system for the doctor course. The capacity of the doctor course is 161 students per year in our Graduate School of Medicine, and 90% of applicants are usually eligible to enter the course. Low scores in the English examination usually exclude student from entering the course.
