研究室概要

名古屋市千種区にある愛知県がんセンター研究所の遺伝子医療研究部と疫学・予防部は、がんに関する疫学研究を実施しています。本研究室では、従来からの生活習慣、環境要因のがんリスクに対する影響を評価する疫学研究のみならず、遺伝的背景のがんリスクへの評価を行う分子疫学、それらのがん予防への応用研究も行っております。また、がん登録を初めとする公的ながんデータを駆使し、がん対策に資するエビデンスを作る研究も行っております。臨床研究グループと共同した臨床疫学研究も数多く実施しております。がんに対する疫学研究組織として、規模・実績ともに、全国でトップクラスです。

研究プロジェクト

当センターでは、1988年から2013年の間、愛知県がんセンター中央病院外来において、初診患者さんを対象に、HERPACC(Hospital-based Epidemiologic Research Program at Aichi Cancer Center)と呼ばれる大規模な研究を実施してきました。がん、非がんの患者さんが混在する集団を対象に、生活習慣、採血を含む疫学調査を行ってきました。本研究は下記に触れる様々な研究プロジェクトの大元となっており、予防、臨床の両方につながる疫学研究を展開しています。

  1. がんの罹患リスクを検討する研究
    がんに罹りやすい人、罹りにくい人の違いにどういう背景があるかを探す研究を実施しています。生活習慣を初めとする環境要因、遺伝子に基づく宿主要因、および両者の交互作用を解明し、それらをがんリスクを評価する事にどう用いるかを検討しています。
    i) がんのリスクに影響を与える環境要因を検討する研究
    ii) がんのリスクに影響を与える遺伝的要因を検討する研究
    iii) 環境要因と遺伝的要因の交互作用を明らかにする研究
    iv) i)-iv)を組合わせた、がんリスク予測に関する研究
    v) がんの個性によるリスク要因の違いに関する研究
    i〜iv)を総合した研究の実例として以下に食道がん・頭頸部食道がんリスクに対する雅飲酒とALDH2遺伝子多型の交互作用を用いたリスク予測の研究の論文リンクを参照 (https://www.ncbi.nlm.nih.gov/pubmed/26862830)。  
    同研究では、HERPACC第2期の症例対照研究で、飲酒とALDH2遺伝子型、喫煙、性、年齢情報に基づき頭頸部・食道がんのリスクを予測するモデルを作成しました。モデルが妥当かどうかを、HERPACC第3期の症例対照研究で検証し、再現性があることを確認しました。同研究では、さらに飲酒量とALDH2遺伝子型の組合わせ別に頭頸部・食道がんの罹患リスクの推定を行っています。
  2. がんサバイバーシップ研究  
    がんに罹った後の生存に最も影響を与えるものは治療です。しかしながら、治療に加えてより良い生活習慣、あるいは治療効果をよくする遺伝的背景があるかもしれません。我々は愛知県がんセンター中央病院と協力して、生活習慣や遺伝的背景がどのようにがん患者さんの治療成績と関係しているかを探る検討をしています。将来的には、治療成績が向上するために必要な生活習慣は何か、を提案できるようにしたいと考えています。  本研究の実例として、葉酸摂取と頭頸部がんの予後に関する研究を紹介します。 (https://www.ncbi.nlm.nih.gov/pubmed/21460376)  
    同研究では、愛知県がんセンターで頭頸部がんと診断され治療を受けた患者さん437名の治療後の予後が、葉酸摂取量(診断前の食事パターンに基づく)が高い患者さんでは良好である、という結果を得ました。今後、がんの種類を広げて、治療後の生活習慣などの治療成績への影響を調べる研究を展開していく予定です。
  3. 国際共同研究への参画  
    がんは日本だけで起きている訳ではなく、世界中で起きています。その原因は、日本人特有なものもあれば、そうではなく人類共通のものである可能性もあります。これらを明らかにするために世界中の研究者と共同して研究するプラットフォームに参画しています。現在参画している国際コンソーシアムは以下の通りです。

    これらの研究を通じて、これまでにN Engl J Med、Nature Geneticsなどに成果を発表しています。
    本国際共同研究の例として、二つの論文リンクを紹介します。  
    一つ目はvii)のアジアコホートコンソーシアムを通じた研究です。
    https://www.ncbi.nlm.nih.gov/pubmed/21345101)本検討では、アジア人を対象として実施された19のコホート研究をプールし、Body Mass Index の総死亡、癌死亡、循環器死亡への影響を検討しました。
    二つ目は、iii)の乳がんに関する研究です。
    https://www.ncbi.nlm.nih.gov/pubmed/23535729)本検討は、BCAC,に参画する疫学研究を対象に全ゲノム関連解析研究を実施したところ、乳がんリスクに影響を与える41遺伝子座見つかった、という研究です。
  4. 乳がんに対する環境要因、宿主要因情報を用いた予防介入研究  
    乳がんはわが国の女性の罹患第一位のがんです。これまでに数多くの研究が環境要因、宿主要因それぞれに関して乳がんリスクとの関連を示してきましたが、それらを組み合せて評価されたことは国内では皆無でした。我々の研究グループでは日本医療研究開発機構AMEDより「個人の生活習慣等の環境要因と遺伝的リスクを考慮した科学的根拠に基づく効率的な乳がん予防法の開発研究」の課題名で支援頂き、国立がん研究センター、鹿児島大学、国立保健医療科学院、徳島大学、大阪大学と連携し、①環境要因と宿主要因を用いた乳がんリスク予測モデル構築、②リスク予測モデルを用いた予防介入試験、③リスク予測を考慮した予防の経済効果予測、④リスク予測を用いた予防法の公衆衛生学的効果の予測を行う研究を実施しています。
  5. 日本人を対象としたがん予防法の検討
    日本で数多くの疫学研究が実施されて来ましたが、それらを総合的に評価し、日本人のためのがん予防はどうあるべきか、という事を評価する事も重要であると考えています。我々は国立がん研究センターのグループが中心となって推進している「科学的根拠に基づく発癌性・がん予防効果の評価とがん予防ガイドライン提言に関する研究」班に参画し、積極的に日本人のがん予防に資するエビデンスの構築に関っています。

  6. 日本他施設共同コーホート研究 (J-MICC研究)  
    当センターは日本多施設共同コーホート研究、通称J-MICC研究に1研究サイトとして参画しております。(http://www.jmicc.com/)J-MICC研究は日本で初めての本格的且つ大規模な分子疫学コホート研究です。
  7. 3府県コホート研究 
    愛知  名古屋市千種区と豊田市足助町において1985年に立ち上げられた住民ベースのコホート研究です。対象者数は33,529名であり、他の3府県コホート(大阪府、宮城県)と共同し、現在までに数多くの成果を出してきました。本データを用いてAsia Cohort Consortium、日本人を対象としたがん予防法開発研究にも参画しています。

  8. 地域がん登録データを用いた記述疫学研究  
    愛知県は1962年から「愛知県悪性新生物患者届出要項」に基づき愛知県がん登録事業を実施しています。当センターは1983年より、本事業に対する技術支援を行っています。我々は、愛知県がん登録(http://www.pref.aichi.jp/soshiki/kenkotaisaku/0000002532.html)データのみならず、全国がん罹患モニタリング集計(http://ganjoho.jp/reg_stat/statistics/brochure/monitoring.html)や米国SEER(https://seer.cancer.gov/)等のデータを駆使し、がんの動向を捉えると共に、がん予防の施策につながる研究を実施しています。  
    本課題に関しては、二つ研究論文を紹介します。  
    一つ目はフィルター無しタバコから有りのタバコに推移することが、日本、米国の肺がんの組織型の推移に影響を与えた事を明らかにした論文です(https://www.ncbi.nlm.nih.gov/pubmed/20589676)。  
    二つ目は造血器腫瘍の罹患率の推移を日米の地域がん登録データを用いて比較した論文です(https://www.ncbi.nlm.nih.gov/pubmed/24245986)。  
  9. 臨床疫学研究
    我々は愛知県がんセンター中央病院のみならず、名古屋大学をはじめ国内外の研究機関において実施される臨床研究に疫学・統計の立場で支援を行っています。
